INVITATION TO REGISTER E-MAILS
Those who have sent registration forms should be hearing as soon as tomorrow regarding the invitation to register e-mails.
SOCIAL SECURITY NUMBERS
You do not need to include S.S.#s on student registration materials.
INSURANCE CARD
Please send a photo copy of the front and back of any registrants insurance card. These can be scanned and sent with registration materials.
REGISTRANTS WITHOUT HEALTH INSURANCE
Will need to complete a “Guaranty of Payment for Medical Treatment”. Apparently President Obama isn’t underwriting NYC medical claims.
BACKGROUND CHECKS
All adults will need to have a background check. According to NYC they will provide this free of charge. They will not accept your own regardless of how recent.
ADULT SPONSORS
Again, if you want to attend NYC please send me an e-mail by Nov. 29th with your name and e-mail address. The adults will not be held to the same registration deadline and there will not be a cost increase for later registrations.
